Simply fill them out before your next appointment and you’ll be able to see Dr. Gufford in no time! If you’re not able to print the paperwork, we can just as easily fax or mail the forms to you as well.

Our plan offers similar benefits to dental insurance but with fewer restrictions and offers a viable way for people to get their essential dental care.
